Recording: 8/18/26 Best Practices in Team-Based Care-Pharmacy Diabetes QI Learning Collaborative

Last updated by Der K.

Notes:

This session featured Dr. Kelley Sanzen, along with Dr. Stephen Kogut from the University of Rhode Island College of Pharmacy, Morgan Leonard and Joe Honig from Clinica Esperanza, and Dr. Bradford Pease from Medical Associates of Rhode Island. Together, they shared two years of work, data, and hard-won lessons from CTC-RI's pharmacy and diabetes quality improvement initiative — including meaningful gains in eye exam screening, kidney health evaluation, and glycemic control across seven Rhode Island primary care practices.
